Perimenopause is the transition period, often beginning 8 to 10 years before menopause, typically in a woman’s 40s. Oestrogen levels don’t decline in a straight line during this phase; they fluctuate, which explains why signs of menopause like hot flushes and mood shifts can feel so erratic. Periods become irregular, sometimes heavier, sometimes lighter, sometimes arriving weeks apart.

Women who have a uterus require progesterone alongside oestrogen to protect the uterine lining, while women who’ve had a hysterectomy are typically prescribed oestrogen alone.

Vaginal Estrogen for Genitourinary Symptoms

For genitourinary symptoms specifically, low-dose vaginal oestrogen carries a different risk profile from systemic HRT and is considered very safe for the vast majority of women, including many who can’t use systemic hormone therapy. It’s available as a cream, pessary or ring, and current evidence supports long-term use.

Perimenopause can generally be identified through symptom history alone in women over 45. A Follicle-stimulating hormone (FSH) blood test may be used in some cases, though FSH fluctuates during perimenopause, and a single reading isn’t always a reliable indicator.

Women in their 40s are regularly misdiagnosed with anxiety or depression before a hormonal cause is considered. If that happens, asking specifically about perimenopause is a reasonable and appropriate step.
